SNOMED codes in Hero forms

Hero uses SNOMED CT codes in the background to make sure the information collected in forms or sent to patients is stored in a standardised way that works across NHS systems.

You don’t need to do anything with these codes yourself – they’re automatically added so that records can be shared consistently into EMIS, SystmOne and other clinical systems.

Top-level codes

Every form or action in Hero has one main SNOMED code attached. For example:

Question-level codes

Some form questions (like observations) also carry their own SNOMED code. This helps ensure that clinical data is stored correctly in the patient record. For example:
