A patient journey through Care Navigation ends in a Care Navigation Option. Care Navigation Options can take a variety of formats:
A request - A request is a customisable structured form. Once submitted request data is visible within the Hero workflow.
A URL - This can be any website you wish to direct patients to
A 3rd party application - Hero allows you to direct patients to a variety of 3rd party platforms including Klinik and Joy
An appointment booking - A link to book a specific appointment
How do I add a new Care Navigation Request Template to a Pathway
To add a new Care Navigation request template, first click 'Edit' on the Pathway you want to add the Care Navigation Request to.
Then, click 'Add option' and select the Care Navigation Option type you wish to add. In the example video below, we are adding the 'Medical request' request type.
Next, optionally add an inbox assignment so you can specify which team or admin the requests are assigned to in the inbox.
Then, choose a submission type and give it a description. You can also limit how many submissions of this type can be made per day and define whether the request type is available in or out of hours.
Finally, by selecting 'Only accept submissions from registered patients', you can prevent submissions from patients not registered on PDS with your practice's ODS code. If you do not tick this box, anybody that has an NHS number will be able to submit a request to your practice.
How do I create or edit a request form?
To create or edit a request template, view the 'Request templates' table, and then select either 'Add request' or under options against an existing request select 'Edit':
You'll then be presented with the ability to build or customise your request template form. On each form, you can edit:
The request details
Internal facing name - Visible only to you and your administrative users
Internal facing description - Visible only to you and your administrative users
SNOMED code - This code is saved to the patient record if you save the submission to the record
The patient facing details
Name
Description
Confirmation message - Shown upon successfully submitting the request
You can the add field types or edit existing field types, including free text entry, radio buttons and check boxes:
You can re-order the questions by simply dragging and dropping.
Once you've added the fields you require, save the form, and this will then be available to be added as a Request to your submission.
How to add an appointment booking link?
To create an appointment booking link, start by adding a new pathway option and select Add booking link. At this point, you can choose to either select one of your pre-built booking links or create a new one.
To create a new booking link, select Create, this will open up the booking link builder where you can select the appointment types, locations (and optionally the practitioners) you wish to make available online. Then provide a name for your link, this will be displayed to patients on the care navigation portal. Finally, select if you want this link to be available In hours and/or Out of hours, then click Save to pathway.
Make sure to click Update pathway, to ensure your changes are saved.
