A questionnaire in Hero is a formattable form that can be sent to patients to collect information.
How do I add a questionnaire to a message?
Once you are in the message builder you will see the 'Insert questionnaire link' button, when clicked you can then select from the questionnaires in the dropdown and select the questionnaire that you would like to send to your patient then click Submit.
How do I create a questionnaire or update an existing questionnaire?
To create or edit a questionnaire, navigate to settings then Questionnaires and then click questionnaire templates, here you can edit, archive or add questionnaires.
