Hero makes it easy for healthcare professionals to submit requests on behalf of their patients. Follow the steps below to ensure accurate submissions.

How do I submit on behalf of a patient?

  1. Navigate to the Hero Homepage

    • Start by logging into Hero and accessing the homepage.

  2. Click "Submit Patient Request"

    • Locate the "Submit Patient Request" button and click it to initiate the process.

  3. Select the Patient

    • Choose the patient for whom you are submitting the request from the list or search by name.

  4. Choose the Pathway and Request Type

    • Select the appropriate pathway that aligns with the patient’s needs.

    • Specify the type of request you’re submitting.

  5. Fill Out the Form

    • Complete all required fields in the form, providing accurate and detailed information.

  6. Submit the Request

    • Review the information for accuracy.

    • Click "Submit" to finalise and send the request.

Now the request will be in your Hero inbox, ready for review.

How do I make a request template not visible for the patient but with the ability to submit on behalf of patient?

Once you have created your request template, you can add it to your care navigation pathway. To make it visible to admins but not your patients you will need to 'Suspend access' by ticking the box next to the request type and then click 'Update pathway'.
