How do I submit a request?

In this article we will take you through submitting a request at your practice.

Your practice will provide you with a number of ways to submit a request that is either for a medical issue or an administrative query for example requesting a doctors letter.

Once you've selected the request you want to submit, you will need to either continue using your NHS Login or have the following information to hand: NHS number & date of birth or their name, date of birth and postcode.

When you have provided this information you are then able to fill in the details provided in order to submit the request and once submitted, the practice will be in touch with any next steps.

Can I submit a request for somebody else?

Yes you can. Once you've selected your request you can choose whether you're submitting for yourself or for somebody else.

All you need is the details of the patient in order to submit the request. This could be their NHS number & date of birth or their name, date of birth and postcode.

You will need to have access to the contact details for the person you're submitting the request for.
